nAs a Physical Therapist , you’ll contribute to our success in the following ways:
nnnEvaluate patients and develop individualized treatment plans.
n
nnProvide physical therapy services in the home setting.
n
nnMonitor patient progress and adjust care plans as needed.
n
nnCollaborate with physicians, caregivers, and the interdisciplinary care team.
n
nnComplete timely and accurate clinical documentation.
n
nnSupervise therapy assistants and support staff, as appropriate.
n
n nWhat is Required?
nnnGraduate of an American Physical Therapy Association (APTA) approved physical therapy program
n
nnCurrent, unrestricted PT license valid for the state of work
n
nnMinimum one (1) year of experience in a healthcare facility
n
nnMust possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
n
nnAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
n
