nPhysical Therapist (PT), Home Health, PRN, 1099

nEssential Functions & Responsibilities of the Physical Therapist (PT):
nPerform comprehensive assessments and evaluations of patient care needs, including: functional status and mobility, endurance and stamina, pain levels and other medical conditions
nDevelop and implement individualized treatment plans as a Physical Therapist (PT)
nProvide skilled interventions to improve patient well-being and functional outcomes
nMonitor and evaluate patient progress, adjusting care plans as needed
nDeliver thorough education and training to patients and caregivers
nParticipate in staff and administrative meetings
nQualifications for the Physical Therapist (PT)
nActive license from the Florida Board of Physical Therapy
nValid Florida driver’s license, active vehicle insurance, and reliable transportation
nAbility to pass AHCA Clearinghouse background screening (
nCurrent in-person CPR certification
nPhysical ability to perform job duties including standing, bending, lifting, and assisting patients/equipment
nStrong customer service, communication skills and effective listening
nEfficient and timely documentation
nExperience Preferred for the Physical Therapist (PT)
nMinimum 1 year of Physical Therapy experience
nMinimum 1 year of Home Health experience
nCompensation for the Physical Therapist (PT)
nPay is commensurate with Physical Therapist (PT) experience, Home Health background, and documentation proficiency
