What You'll Do Provide skilled physical therapy services in patients' homes following the physician's plan of care.
Evaluate patient mobility, strength, balance, and functional abilities.
Develop individualized treatment plans to help patients reach their rehabilitation goals.
Educate patients and caregivers on exercises, mobility techniques, and safety strategies.
Recommend adaptive equipment and home modifications to improve safety and independence.
Collaborate with physicians, nurses, and the interdisciplinary care team to ensure exceptional patient outcomes.
Complete accurate and timely clinical documentation.
Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations and company policies.
Monitor patient progress and adjust treatment plans as needed.
Qualifications Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA).
Current Physical Therapist (PT) license in the state of Tennessee.
Minimum of two (2) years of experience in acute care or a similar clinical setting preferred.
Current CPR and First Aid certification.
Valid driver's license, reliable transportation, and current automobile insurance.
