Day (United States of America) Job Summary:

n The physical therapist assesses, plans, organizes and participates in rehabilitative programs that improve mobility, relieve pain, increase strength and decrease or prevent deformity of patients suffering from disease or injury. Services are provided across the full continuum and rendered in a cost conscious, quality focused and customer oriented manner. Compliance with all applicable regulatory standards is also required in this role.n n Core Responsibilities and Essential Functions:

n Treatment Planning and Provision of Caren * Implements the physical therapy treatment plann * Selects appropriate treatment activities to progress patient towards goals.n * Revises goals/plan of care with patient/family inputn * Provides ongoing patient/family education and trainingn * Provides discharge instructions, follow-up and referral to community resources as appropriaten * Acts as a referral source for staff, families, physicians on services and equipment related to rehabilitation services.n Patient Assessmentn * In collaboration with the physician, responsible for the establishment and ongoing evaluation of treatment programs, plan of care, goals and discharge plans for the patients as related to the physical therapy needs of the patient.n * Establishes a plan of care in conjunction with the patient and/or family members.n * Establishes realistic discharge plan incorporating patient and/or family member goals.n Documentationn * Documents the evaluation and plan of care according to departmental guidelinesn * Documents each treatment session according to departmental protocolsn * Completes and submits family education, discharge and other required documentation within facility and program guidelinesn General requirementsn * Adheres to ethical business practices by striving to perform in a manner that conforms to the highest standards of ethical behavior, integrity and honesty.n * Embraces diversity and demonstrates the ability to work together.n * Maintains confidentiality of all patient information.n * Maintains attendance/punctuality in accordance with system policy and procedure.n * Adheres to departmental dress code policy, including wearing and displaying ID badge appropriately.n * Communicates information effectively both verbally and in writing with all team members.n * Attends and participates in staff meetings, required in services, process improvement activities, and other meetings as requested.n * Organizes workload to complete responsibilities in an appropriate and timely manner.n * Utilizes materials and equipment in a safe and cost-effective manner.n * Observes infection prevention precautionsn Customer Servicen * Strives to help lead the industry through vision, technology, innovation and customer service.n * Strives to make a difference in the lives of our employees and patients.n * Consistently applies AIDET principles in all interactions with patients.n * Encourages a positive environment and experience for co-workers and patients. Doctorate Physical Therapy or Masters Physical Therapy or Bachelors Physical Therapy

n Required Minimum License(s) and Certification(s):

n All certifications are required upon hire unless otherwise stated. PT - Physical Therapist

BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
