nAlwaysNear Home Care LLC is looking for a dedicated and compassionate Physical Therapist (PRN) to join our growing team in Atlanta, GA. nResponsibilities:
nConduct comprehensive physical therapy evaluations and assessments for home care patients
nDevelop and implement individualized treatment plans to restore function and improve mobility
nProvide therapeutic interventions including therapeutic exercise, manual therapy, and neuromuscular re-education
nMonitor and document patient progress, adjusting care plans as clinically appropriate
nEducate patients and caregivers on home exercise programs and injury prevention strategies
nCollaborate with interdisciplinary home health team members to ensure coordinated patient care
nMaintain accurate and timely clinical documentation in compliance with regulatory standards
nRequirements:
nValid Georgia Physical Therapist (PT) license in good standing
nGraduate of an accredited Physical Therapy program (DPT or equivalent)
nHome health or outpatient physical therapy experience preferred
nExcellent communication, interpersonal, and patient education skills
nReliable transportation and valid driver's license for home visit travel throughout Atlanta, GA
nAbility to work independently and manage a flexible PRN schedule
