As a Home Health Physical Therapist you'll provide skilled, one-on-one therapy services in the comfort of patients' homes, helping them regain strength, mobility and independence. You'll assess patient needs, develop individualized treatment plans, deliver evidence-based interventions and educate patients and caregivers to support safe recovery at home.

Qualifications
Current licensure as a Physical Therapist in the State of Michigan
Graduate of an accredited physical therapy program
At least one year of clinical experience (home care experience preferred)
Valid driver's license and reliable transportation
Ability to work independently and manage time effectively
