Full Time Physical Therapist (PT)
CommuniCare Rehab, a member of the CommuniCare Family of Companies has a Full Time opening for a Physical Therapist (PT) at Cedars Healthcare Center, where our therapists are In-House Team Members. This is an exciting opportunity to make a direct impact with our residents through therapy. CommuniCare is a rapidly growing company, with 170 centers across IN, MD, MO, OH, PA, VA, WV.

Qualifications:
A Bachelor's, Master's, or Doctorate Degree in Physical Therapy from an Accredited Program.
Hold a valid or pending State Physical Therapy License
Demonstrate competency in therapy treatment, evaluation principles, techniques, and materials & equipment utilization. Attainable productivity expectations.
Job Responsibilities include, but are not limited to:
Evaluate, Review, and Screen patient's/resident's medical charts, evaluations under physician order and develop a treatment plan in accordance with patient's needs.
Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
Maintain accurate billing information in accordance with facility procedures daily.
Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
