nAbout the Role:
nAs a physical therapist, the primary focus is on facilitating the restoration of individuals' physical well-being. This involves conducting comprehensive assessments, developing personalized treatment plans that encompass therapeutic exercises and interventions, and collaborating seamlessly with interdisciplinary healthcare teams. Central to the position is the continuous monitoring of patient progress, alongside effective advocacy for their overall health. Proficiency in communication and meticulous documentation is crucial, coupled with a dedication to ongoing professional development, for sustained excellence in this role.
nResponsibilities:
nDevelop and implement physical therapy treatment plans for clients
nProvide physical therapy assessment and evaluations
nWork with patients to improve their range of motion and strength
nEducate and counsel patients and their families on physical therapy treatments
nDocument all patient care services and progress
nMaintain patient records
nConduct thorough assessments of patients' physical conditions and functional abilities.
nDevelop individualized treatment plans based on clinical assessments and physician recommendations.
nImplement evidence-based physical therapy interventions to improve mobility, strength, balance, and overall functional independence.
nMonitor and document patient progress, adjusting treatment plans as necessary.
nCollaborate with physicians, occupational therapists, and other healthcare professionals to ensure comprehensive and coordinated patient care.
nProvide education to patients and their families on home exercise programs and self-management techniques.
nRequirements:
nLicensed Physical Therapist in the state of Florida
nAt least 1 year of experience in physical therapy
nAt least 1 year of experience working in a Home Health environment
nExcellent communication and interpersonal skills
nStrong problem-solving and critical thinking skills
nAbility to lift heavy objects
nExcellent interpersonal and communication skills.
nAbility to work independently and collaboratively within a multidisciplinary team.
