nElara Caring is seeking a Physical Therapist to serve patients throughout the Danbury, Connecticut territory. In this field-based role, you will help patients regain strength, mobility, and independence in the place they feel most comfortable: home.
nYou will have the independence to manage your patient visits while receiving support from an experienced clinical team.

n nWhat You’ll Do
nnnEvaluate patients’ mobility, strength, safety, and functional needs
n
nnDevelop and implement individualized treatment plans
n
nnHelp patients improve mobility and safely perform daily activities
n
nnEducate patients and caregivers on exercises, equipment, and safety techniques
n
nnMonitor patient progress and adjust treatment plans as needed
n
nnCommunicate changes in condition to physicians and members of the care team
n
nnComplete accurate and timely clinical documentation
n
nnCollaborate with nurses, physicians, therapy professionals, and other team members
n
nnSupport patients in reaching their highest possible level of independence
n
n nWhat You’ll Bring
nnnGraduate of an American Physical Therapy Association-approved physical therapy program
n
nnActive and unrestricted Connecticut Physical Therapist license
n
nnAt least one year of experience in a healthcare setting
n
nnStrong communication, organization, and patient-care skills
n
nnAbility to work independently in a home-based environment
n
nnWillingness to travel throughout the assigned Danbury territory
n
nnMust have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
n
nnMust be able and willing to travel 50%
n
nnAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
n
n nHome health experience is valuable, but candidates with transferable clinical experience are encouraged to apply.

nThe base salary for this position is $100,000 to $115,000 annually, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
