Physical Therapist Opportunity
Flagstar Nursing is seeking Physical Therapists (PT) for Skilled Nursing Facilities!
Now staffing Physical Therapists for the rehab department in state-of-the-art SNFs throughout NYC and Long Island.
Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.
Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)
Physical Therapist Job Duties:
Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
Supervise PTAs daily.
Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
Monitor and document patient progress with the use of innovative EMR software.
Physical Therapists Employed by Flagstar Rehab Will Receive:

Physical Therapist Requirements:
Current NYS PT License and Registration
CPR Certification
EMR Competence
Meets all State and Federal requirements for US employment in a Nursing Facility
Malpractice/Liability Insurance (if applicable)
