Job Description
Physical Therapist
Schedule
8-hour shifts.
Assignment Details
Contract length: 13 weeks. Guaranteed hours: 40 per week.
Requirements
Current State License as a Physical Therapist - Required
Proof of current CPR certification - Required
Current driver's license with reliable transportation and insurance - Required
1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
Previous home care experience - Preferred
Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required
Responsibilities Perform clinical assessments to identify patient needs and appropriateness of care.
Document and complete a Plan of Care (POC), progress notes, and other clinical record documentation.
Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on physician referrals.
Assist and instruct patients and families in transfer techniques and the use of therapeutic equipment.
Provide patient and family education regarding therapeutic procedures.
Oversee other team members in delivering patient care consistent with patient needs.
