Physical Therapist - Home Health
Schedule
Day shift, 5 days a week, 8-hour shifts from 08:00 to 16:30.
Assignment Details
Full-time position with a productivity expectation of around 25 units for 7 visits per week.
Requirements
Minimum of 1 year of experience; first-time travelers accepted.
Active BLS certification.
Georgia Physical Therapy license or compact Physical Therapy license accepted (verification required).
Experience in home health with a focus on geriatrics, strokes, and orthopedics preferred.
Familiarity with Home Care Home Base (HCHB) preferred; if not, completion of an online HCHB course is required (cost $250, must be paid upfront by traveler).
Responsibilities Provide physical therapy services to adult and geriatric patients in a home health setting.
Assess patient conditions and develop individualized treatment plans.
Monitor patient progress and adjust treatments as necessary.
Educate patients and families on rehabilitation strategies and exercises.
Document patient care accurately and timely.
Collaborate with other healthcare professionals to ensure comprehensive patient care.

Compliance / Notes
Full compliance required to start on time; non-compliance may lead to a two-week delay due to orientation scheduling. If holidays occur during the assignment period, the traveler may be required to work them as specified.
