Job description
PT Home Health
Day 5x8-Hour (08:00 - 17:00), with 1-2 weekends per contract.
Skilled home visits of 6-8 per day.
Requirements
1 year home health experience required.
BLS certification required.
Must possess a valid state license.
OASIS training preferred.
Must have a valid driver's license and personal vehicle for home visits.
COVID vaccination or approved exemption required; flu shot or declination must be on file with mandatory mask wearing at all times while at work.
Experience with assessment, documentation, and knowledge of equipment and devices including gait belts, walkers, and more.
Skills in ortho, neuro, home exercise programs, medication management, and patient/family education.
Responsibilities
Perform assessments and documentation for skilled home care patients.
Develop and implement individualized home exercise programs.
Provide education to patients and families regarding health management.
Identify signs and symptoms of abuse and implement safety and injury prevention measures.
Maintain compliance with billing practices.
Utilize medical equipment and devices appropriately during patient care.
