n Physical Therapist (PT) – Home Health Case Managern
n Location:n n Chicago – South Side (servicing ZIP codes 60620, 60628, 60643 and surrounding areas)n
n Schedule:n n Day shift with rotating weekends and holidaysn
n Sign-On Bonus:n
n Eligible for up ton n $10,000 sign-on bonusn n for external candidates. Must be actively working in the same role at the time of payout.n

n Provide comprehensive physical therapy assessments and develop individualized care plansn
n Deliver in-home therapy focused on improving mobility, safety, and independencen
n Serve as a case manager, coordinating care with physicians and interdisciplinary teamsn
n Educate patients and families to support recovery and long-term outcomesn
n Travel throughout the assigned North Side Chicago territoryn
n What You’ll Needn
n Licensed Physical Therapist in Illinois (or eligible)n
n Minimum 1 year of clinical experience (or strong clinical affiliation experience)n
n Experience with home-based therapy equipment and assistive devicesn
n Strong communication, documentation, and time management skillsn
n Ability to work independently while collaborating with a broader care teamn
