Physical Therapist - Home Health
Schedule
Every 3rd weekend required.
Assignment Details
Home skilled visits - 6-8 per day, with a minimum of 6 points per day, at the manager's discretion may increase by 1 point.
Requirements
2 years of home health experience required.
Valid state license.
Certifications: BLS.
Must have a valid driver's license and car for home visits.
COVID vaccination required or exemption form on file.
Flu shot required or declination form; mandatory mask wearing while at work.
OASIS training preferred.
Experience with various medical equipment and devices.
Responsibilities Conduct patient assessments focusing on cardiopulmonary, neurological, orthopedic, and oncology specialties.
Develop and implement home exercise programs tailored to patient needs.
Educate patients and their families on medication management and signs of physical, sexual, and psychological abuse.
Document patient progress and care in required format.
Promote safety and injury prevention strategies during home visits.
