Job description
Physical Therapist – Home Health
Self-scheduled visits with a minimum of 6 skilled visits per day, with every 3rd weekend required.
Contract length and guaranteed hours will be determined based on the needs of the HomeCare office.
2 years of home health experience required
Current state license and BLS certification required
Valid driver's license and car for home visits
OASIS training preferred
COVID-19 vaccinated or exemption form on file
Flu shot or declination required; mandatory mask wearing at work
Conduct assessments and develop patient care plans
Implement and monitor home exercise programs
Educate patients and families on care plans, medications, and safety
Document patient progress and treatment outcomes
Utilize various medical equipment as needed
Recognize signs and symptoms of abuse and implement safety measures
