Physical Therapist - Home Health
Schedule
Self-scheduled visits with minimal 6 skilled home care visits per day and 1-2 weekends per contract.
Assignment Details
Contract length and guaranteed hours are based on the needs of the HomeCare office.
Requirements
1 year of home health experience required
Valid driver's license and personal vehicle required for home visits
BLS certification required
State license required
OASIS training preferred
COVID-19 vaccination required or exemption form on file
Flu shot or declination required
Mandatory mask wearing at all times while at work
Responsibilities Conduct patient assessments in cardiopulmonary/circulatory areas
Document treatment plans and patient progress
Utilize equipment and devices, including gait belts and walkers
Educate patients and families on exercises and safety measures
Monitor signs and symptoms of physical and psychological conditions
Ensure safety and injury prevention during home visits
