Pay Rate: $ 2,000 To $ 2,208 Per week
Schedule: Day Shift

About the Role
We are seeking an experienced Physical Therapist (PT) - Home Health for a 13-week contract in California. The Physical Therapist will provide skilled home-based rehabilitation services while prioritizing Start of Care visits, supervising Physical Therapy Assistants, and maintaining timely clinical documentation and communication.
The position follows a productivity-based home health model with an expected 30 points per week . The therapist will manage a community-based caseload and travel between patient locations throughout the assigned territory.
Responsibilities
Perform comprehensive home health physical therapy evaluations and prioritize Start of Care visits according to clinical and scheduling requirements.
Develop and implement individualized treatment plans designed to improve patient mobility, strength, balance, safety, and functional independence.
Supervise Physical Therapy Assistants and coordinate therapy services to support continuity and quality of patient care.
Complete mandatory next-day Start of Care reporting and submit clinical notes and other required documentation within 48 hours of each visit.
Maintain timely communication with supervisors, intake teams, and office staff while meeting the expected productivity standard of 30 points per week.
Requirements
Minimum 1 Year of Home Health Physical Therapist Experience Required.
Current, Active California Physical Therapist License Required.
Ability to Independently Manage Home Health Start of Care, Evaluation, Recertification, Resumption of Care, Revisit, and Discharge Visits Required.
Ability to Supervise Physical Therapy Assistants and Maintain Timely Clinical Documentation and Communication Required.
Valid Driver's License and Reliable Transportation Required for Patient-to-Patient Travel; Ability to Work Monday-Friday and One Weekend Per Month Required.
This contract opportunity is suited for an experienced Home Health Physical Therapist comfortable managing a full caseload, supervising therapy assistants, and meeting productivity and documentation expectations.
