PRN Home Health Physical Therapist (PT)
Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health Physical Therapist (PT) to work with us to provide exceptional and individualized care to patients in the Port Charlotte/North Port and surrounding area.

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.
Locations: Home health visits available in Port Charlotte, Venice, Englewood, Rotonda West, North Port, and surrounding areas.
Schedule: PRN; 5-10+ home health visits available per week
What You'll Do
Perform comprehensive patient assessments, including OASIS assessments at required intervals
Develop, initiate, and revise individualized plans of care as patient needs evolve
Deliver skilled occupational therapy services and treatments as outlined in the plan of care
Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need
Active Florida PT license
Minimum of 1 year experience as a licensed Physical Therapist (PT)
Home health care experience strongly preferred.
Strong assessment, critical thinking, and patient education skills
Willingness to participate in weekend and holiday rotations
