Physical Therapist (PT) - Home Health
Pay Range: $130,000 - $200,000 annually (pay per visit)
Job Type: Full Time

Sequoia Home Health is growing and looking for experienced, compassionate Physical Therapist (PT) to join our team!
Job Description
We are seeking a Physical Therapist to provide in-home therapy services. The PT will deliver care in accordance with a physician's plan, helping patients regain mobility and independence.
Responsibilities
Evaluate patients' functional status and develop care plans
Assess home environments and recommend modifications
Provide direct physical therapy treatments
Educate patients and caregivers on therapeutic techniques and equipment
Monitor progress and adjust treatment plans
Collaborate with physicians and interdisciplinary teams
Document clinical notes and progress reports
Participate in discharge planning
Supervise Physical Therapy Assistants per policy and regulations
Qualifications Active California Physical Therapy License
Minimum 2 years of physical therapy experience (home health preferred)
Strong communication and organizational skills
Current CPR certification
Valid driver's license and insured, reliable vehicle
