Physical Therapist (PT) – Long-Term Care
LP Therapy
LP Therapy is seeking a licensed Physical Therapist (PT) to provide skilled therapy services in a Long-Term Care setting focused on restoring mobility, strength, and function.
Responsibilities
Review physician referrals and medical records
Evaluate strength, mobility, and functional ability
Develop and modify treatment plans
Administer therapeutic exercises and modalities
Educate patients and families
Complete timely Medicare documentation
Collaborate with care teams
Supervise PTAs and aides as required
Support quality outcomes and productivity standards
Qualifications
Bachelor's degree in Physical Therapy
Current state PT license
LTC experience preferred
Strong knowledge of Medicare guidelines
Effective communication and teamwork skills
Work Environment
LTC facility; physical demands include lifting and mobility assistance.
