In this role, you will assess participants' mobility and functional needs, develop individualized treatment plans, and provide therapeutic interventions tailored to an adult and geriatric population. You'll collaborate with an interdisciplinary care team to address complex needs and help participants remain as active and independent as possible. Bachelor's degree in Physical Therapy from an accredited program.

Valid California Physical Therapist (PT) license or documented proof of eligibility for California licensure.

One (1) to two (2) years of professional experience.

Ability to obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter.

Strong clinical assessment, treatment planning, and patient education skills.

Ability to provide individualized physical therapy services to adults with varying functional and healthcare needs.

Strong communication and interdisciplinary collaboration skills.

Willingness and ability to travel occasionally as required.

Experience treating older adults with mobility limitations, chronic conditions, or complex healthcare needs.

Experience developing individualized rehabilitation programs focused on mobility, strength, balance, fall prevention, and functional independence.

Ideal Candidate

The ideal candidate is a compassionate, adaptable Physical Therapist with professional adult and/or geriatric experience who understands that maintaining mobility can be essential to preserving an older adult's independence and quality of life. They build trust with their patients, recognize progress beyond the treatment room, and know how to adapt rehabilitation strategies to individuals with complex needs.
