Physical Therapist (PT)

At H2 Health @ Berea, you'll work in a 2,500-square-foot outpatient clinic alongside an experienced therapy team with more than 80 years of combined experience. The clinic serves a diverse general outpatient population and offers programs across orthopedic and musculoskeletal rehabilitation, balance and dizziness, sports medicine, chronic pain, neurological conditions, manual therapy, Workers' Compensation, and other rehabilitation needs.
Your Role
Evaluate patients and develop individualized, evidence-based Physical Therapy plans of care
Treat a diverse outpatient caseload focused on mobility, strength, function, and return to activity
Provide therapeutic exercise, neuromuscular re-education, manual interventions, and functional training
Educate patients on movement, recovery, injury prevention, and home exercise programs
Collaborate with clinicians and support staff to coordinate high-quality care
Track patient progress and adjust treatment plans to support meaningful outcomes
Complete timely and accurate clinical documentation
