Physical Therapist
Fort Worth Physical Therapy, a H2 Health company, is seeking a Physical Therapist to join our Southwest Fort Worth outpatient team. At Fort Worth Physical Therapy, you'll work with a diverse outpatient population and provide individualized, evidence-based care to patients recovering from orthopedic injuries, surgical procedures, sports-related conditions, and other musculoskeletal impairments. Our clinicians focus on building strong patient relationships and developing customized treatment plans that help patients restore mobility, reduce pain, improve function, and return to the activities they enjoy most. Your Role:
Perform comprehensive patient evaluations and develop individualized plans of care.
Deliver evidence-based treatment interventions to improve mobility, strength, balance, function, and overall quality of life.
Treat a variety of orthopedic, post-operative, sports medicine, and musculoskeletal conditions.
Collaborate with patients, families, physicians, and interdisciplinary team members to achieve optimal outcomes.
Educate patients on injury prevention, wellness, and home exercise programs.
Maintain accurate and timely documentation of patient progress and treatment outcomes.
Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
Work within a manageable caseload environment, typically targeting approximately 10-12 patient visits per day for full-time outpatient clinicians.
Contribute to a positive clinic culture focused on patient-centered care and clinical excellence.
