Physical Therapist (PT) Per Diem
Job Summary:
The Palms Care Center, a renowned leader in home health care, is seeking a highly skilled and dedicated Physical Therapist (PT) to join our per diem team in Lauderdale Lakes, Florida. As a Physical Therapist with our organization, you will have the opportunity to work with a diverse range of patients, providing high-quality rehabilitation services in a sub-acute medical care setting. Responsibilities:
Evaluate patients with a wide range of medical conditions, conducting thorough assessments to develop individualized treatment plans
Design and implement effective physical therapy interventions, including exercises, stretches, and modalities, to promote patient recovery and independence
Collaborate with a multidisciplinary team of healthcare professionals to ensure seamless patient care
Maintain accurate and timely documentation of patient progress and treatment plans
Communicate effectively with patients, families, and healthcare team members to address concerns and provide education
Stay current with industry developments and best practices in physical therapy, participating in continuing education and professional development opportunities
Requirements: A Doctor of Physical Therapy (DPT) degree or equivalent education and experience
Current licensure to practice physical therapy in the state of Florida
Proficiency in electronic medical records (EMRs) and ability to learn our organization's specific systems
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines
