Position Summary
We are seeking a dedicated Physical Therapist (PT) to join our in-house rehabilitation team. In this role, you will evaluate residents, develop individualized treatment plans, and provide skilled therapy services designed to improve mobility, strength, function, and independence.
Key Responsibilities Evaluate residents and develop individualized treatment plans
Provide skilled physical therapy services in accordance with physician orders
Monitor resident progress and adjust treatment plans as needed
Collaborate with physicians, nursing staff, families, and interdisciplinary team members
Complete accurate and timely documentation
Participate in care plan meetings and discharge planning
Educate residents and caregivers on exercises, safety, and mobility strategies
Maintain compliance with all regulatory and professional standards
Support positive resident outcomes and quality-of-life goals
Qualifications
Available Sunday through Thursday (weekend availability required)
Degree from an accredited Physical Therapy program
Current South Carolina Physical Therapist license in good standing
Long-term care or skilled nursing experience preferred
Experience with PointClickCare (PCC) and Casamba preferred
Strong clinical, communication, and organizational skills
Must maintain all required continuing education and licensure requirements
