n nResponsibilities
nWhat you will do: Responsibilities listed include but not limited to:
nnnPerform evaluations and develop treatment plans
n
nnRegularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
n
nnPerform discharge evaluations, make appropriate recommendations for safety, and provide referral and education resources
n
nnCommunicate patient’s needs and progress to the treatment team, physician, person receiving services and family members
n
nnSupervision of Physical Therapy Assistants as needed
n
nnCommunicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes
n
n nQualifications
nWhat you will need:
nnnMinimum of bachelor’s degree in Physical Therapy from a college or university with accredited Physical Therapy program
n
nnCurrent, unrestricted license as Physical Therapist by state in which practicing
n
nnCurrent Live CPR Certification
n
nnTwo-step TB skin test required
n
nnProof of Auto Insurance
n
nnDemonstrates knowledge of evidence based therapy techniques
n
nnFamiliarity with assisted living, independent living, geriatrics and/or home health
n
nnCommunicates effectively and professionally in verbal and written interactions
n
nnAbility to lift 50 pounds
n
nnMoving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
n
nnDuties require fine motor skills ,visual acuity, and walking/ standing for extended periods
n
nnAdditional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
n
nnA health screen or examination may be required prior to assignment and periodically thereafter, depending on the specific location policy, local or state regulations to verify that he/she is physically capable of performing assigned duties with or without reasonable accommodations
n
