nLocation: Lufkin, TX and Multiple Locations, Texas Position Type: Part-Time, or PRN
nJob Summary
nWe are seeking a compassionate, dedicated, and licensed Physical Therapist (PT) to join our Home Health & Hospice team. The Physical Therapist is responsible for evaluating patients' physical conditions, developing individualized treatment plans, and providing therapeutic interventions to improve mobility, reduce pain, and maximize functional independence. The PT will work collaboratively with physicians, nurses, and interdisciplinary team members to provide high-quality patient-centered care in the patient's home.
nEssential Job Duties and Responsibilities
nPerform comprehensive physical therapy evaluations and assessments.
nDevelop individualized treatment plans based on physician orders and patient needs.
nProvide skilled physical therapy services in accordance with state and federal regulations.
nEvaluate patients' functional abilities, including mobility, strength, balance, coordination, and endurance.
nEstablish measurable treatment goals and revise plans of care as needed.
nEducate patients and caregivers regarding treatment plans, exercises, safety measures, and disease management.
nProvide therapeutic exercises, gait training, balance training, transfer training, and pain management techniques.
nAssess home environments and recommend adaptive equipment or modifications to improve patient safety.
nComplete all required documentation accurately and timely in the Electronic Medical Record (EMR) system.
nCommunicate effectively with physicians and interdisciplinary team members regarding patient progress and changes in condition.
nParticipate in case conferences, care coordination meetings, and interdisciplinary team meetings when required.
nMaintain compliance with Medicare Conditions of Participation, HIPAA regulations, agency policies, and professional standards.
nSupervise Physical Therapist Assistants (PTAs) when applicable and provide clinical guidance in accordance with state regulations.
nParticipate in orientation programs, competency assessments, and continuing education activities.
nPerform other duties as assigned by the Clinical Supervisor or Director of Patient Care Services.
nQualifications
nEducation
nGraduate of an accredited Physical Therapy program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE).
nBachelor's, Master's, or Doctor of Physical Therapy (DPT) degree required.
nLicensure and Certifications
nCurrent and unrestricted Physical Therapist (PT) license in the State of Texas.
nCurrent CPR certification required.
nValid Texas Driver's License.
nCurrent automobile insurance and reliable transportation required.
nExperience
nMinimum one (1) year of clinical experience as a Physical Therapist preferred.
nHome Health and/or Hospice experience preferred.
nExperience working with geriatric patients is highly desirable.
nKnowledge of Medicare regulations and documentation requirements is preferred.
nKnowledge, Skills, and Abilities

nKnowledge of Home Health and Hospice regulations and standards of practice.
nAbility to educate patients and caregivers effectively.
nStrong documentation and computer skills.
nAbility to establish positive working relationships with patients, families, physicians, and interdisciplinary team members.
nDemonstrates compassion, professionalism, and cultural sensitivity when providing patient care.
nPhysical Requirements
nAbility to stand, walk, bend, lift, and transfer patients as necessary.
nAbility to frequently lift up to 50 pounds with or without reasonable accommodation.
nAbility to travel to patients' homes within assigned service areas.
nAbility to perform patient assessments and therapeutic interventions in various home environments.
