Job description
Sanford Health is seeking a Physical Therapist to plan and deliver rehabilitation services across post-acute, long-term care and home health settings. You will assess patients, develop therapeutic plans, and work to restore mobility and reduce pain in a patient-centered approach.
Responsibilities include conducting evaluations, documenting progress, educating patients and families on treatment plans and exercises, and coordinating care with a multidisciplinary team.
