Job description
Country Hills Post Acute is seeking a dedicated Staff Physical Therapist to evaluate residents, develop and implement individualized treatment plans, and supervise PT aides in a skilled nursing setting.
You will coordinate with physicians, nursing staff, and families to communicate progress, goals, and discharge plans while ensuring compliant documentation and ongoing education for staff.
