n-Physical Therapist -Outpatient * - Rhinebeck, NY - Full Time*
nMon/Wed/Fri 7:45-3:45pm; Tues and Thurs 10:45-6:45pm; occasional weekend day
nnnn nPurpose: **
nThe Physical Therapist will be responsible for providing comprehensive physical therapy services to patients in an outpatient setting. This role involves performing therapy evaluations, developing treatment plans and goals, and applying interventions in accordance with the Standards of Practice of the profession, applicable state licensure regulations, and hospital policies and procedures. Population served is based on scope of services in the department. Potential for rotation to alternative rehabilitation settings may exist based on operational needs.

nEssential Responsibilities
nnnConducts comprehensive therapy evaluations, collaborates with patients to develop individualized treatment plans, and establishes treatment goals to ensure optimal therapeutic outcomes.
n
nnApplies evidence-based practices and adheres to APTA guidelines to independently make clinical decisions within the scope of expertise, while seeking guidance when appropriate to ensure high-quality care and effective performance of duties.
n
nnAdheres to the regulatory guidelines for the provision of care, all applicable billing regulations and statutory guidelines
n
nnDelegates appropriate tasks to assistants, aides, and support personnel. Supervises students as assigned.
n
nnCollaborates with other disciplines to provide coordinated interdisciplinary patient care.
n
nnCoordinates care with the patient and will involve caregivers, as appropriate.
n
nnParticipates in educational in-services, meetings, and performance improvement activities.
n
nnCompletes all documentation within established time frames.
n
nnProvides care to special populations including Total Joint Replacement, Spine, and Stroke as per hospital protocols.
n
nnMaintains and models the organization's values.
n
nnDemonstrates regular, reliable, and predictable attendance.
n
nnPerforms other duties as assigned.

n nEducation and Experience Requirements:
n· Doctoral Degree or equivalent combination of education and related experience
nLicense, Registration, or Certification Requirements:
n· PT License for applicable state (NY or CT), temporary licensure may be considered at the discretion of departmental leadership.
n· BLS certified
