Physical Therapist (Full-Time) - Join Our Team at Franklin County Rehab Center

As a Physical Therapist on our team, you'll play a vital role in helping our patients recover and regain their independence. We're seeking a highly skilled and compassionate Physical Therapist to join our team of healthcare professionals at our state-of-the-art facility in St. Alban's City, Vermont, USA .
About the Role:
As a Physical Therapist at Franklin County Rehab Center, you'll work closely with our interdisciplinary team to develop and implement individualized treatment plans for our patients. You'll work in a dynamic and supportive environment, with access to the latest treatment technologies and equipment. Your responsibilities will include:
Conducting initial evaluations and assessments to identify patient needs and goals
Developing and implementing comprehensive treatment plans, including physical therapy interventions, modalities, and exercises
Collaborating with other healthcare professionals to ensure seamless care coordination and communication
Providing education and support to patients and their families to promote independence and self-care
Maintaining accurate and timely records of patient progress and treatment plans
