Physical Therapist - Senior Care / PACE Program
nSchedule: Monday-Friday, 8:00 AM-4:30 PM
nPay range: $90,000 - $110,000
nJob Type: Full-Time

nAbout the Opportunity
nWe are seeking a compassionate and experienced Physical Therapist to provide evaluation and treatment services to adults age 55 and older. This role focuses on helping participants maintain or improve mobility, safety, independence, and quality of life through individualized physical therapy services.
nThe Physical Therapist will work closely with participants, families, caregivers, medical providers, nursing and assisted living staff, and an interdisciplinary care team to develop and implement comprehensive Plans of Care.
nThis is an excellent opportunity for a Physical Therapist who enjoys working with older adults and values collaborative, patient-centered care.
nKey Responsibilities
nScreen and evaluate participants for acute, restorative, or maintenance rehabilitation needs.
nDevelop, implement, and update individualized physical therapy treatment plans in accordance with physician orders and the participant's Plan of Care.
nPay Details: $69.00 to $70.00 per hour
