As a Home Health Physical Therapist , you are a critical part of the care team - helping patients safely recover, regain mobility, and maintain independence where they feel most comfortable: at home.

This is a highly autonomous, relationship-driven role where you can truly see the impact of your work every day.

What You'll Do

Deliver Patient-Centered Care

* Evaluate and assess patients' functional status, mobility, strength, and rehabilitation potential

* Develop and implement individualized care plans in collaboration with physicians and the care team

* Provide hands-on therapy to improve mobility, strength, balance, and independence

* Educate patients and caregivers on treatment plans, exercises, and safety strategies

Drive Outcomes & Recovery

* Monitor patient progress and adjust treatment plans as needed

* Support safe transitions of care and contribute to discharge planning

* Identify additional patient needs and collaborate with interdisciplinary teams

Collaborate & Communicate

* Partner closely with nurses, therapists, and physicians to ensure coordinated care

* Document patient care and progress accurately and timely within the EMR

* Participate in case conferences and team meetings

Lead & Support

* Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines

* Provide guidance and education to caregivers and support staff

What You Bring

* Graduate of an accredited Physical Therapy program (DPT or Master's)

* Current PT license in the state of practice

* Minimum 1-2 years of experience (home health experience preferred)

* Strong clinical assessment and critical thinking skills

* Excellent communication and patient education abilities

* Current CPR certification

* Valid driver's license, reliable transportation, and auto insurance
