Job description
Outcome HC is seeking a dedicated Full-Time Physical Therapist for a Skilled Nursing Facility in New Jersey. You’ll evaluate residents, design treatment plans to improve mobility, and work with an interdisciplinary team to restore function and quality of life.
New grads are welcome and will receive mentorship and training; SNF/geriatrics experience is a plus. You will document progress in the EMR and participate in care plan meetings to support residents and families.
