Job description
Chatham Hills Subacute Care Center is actively seeking an Experienced Physical Therapist for our facility in New Jersey. The PT will assess patients requiring rehabilitation, develop treatment plans, and set achievable goals under physician direction.
You will document patient progress, educate families, and collaborate with the care team to deliver patient-centered therapy. Ideal candidates will have an accredited PT degree, current licensure, and experience in long-term or geriatric care.
