Minimum Requirements
* Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
* Active MA Physical Therapist license required to start the assignment.
* BLS certification from AHA or ARC may be required.
* Home Health experience preferred. Candidates with Hospital or SNF experience will be considered.
* Experience using electronic medical records. Familiarity with Homecare Homebase preferred.
* Ability to meet productivity target of 6 points/day (30 points/week).
* Valid driver's license and reliable transportation for patient visits within a manageable territory.
* May consider a PT/COTA pair; COTA applicants may be considered when submitted with a PT.

* Facility type: Home Health.
* Shift name: Days.
* Schedule options: 5x8 (Monday-Friday) or 4x10s.
* Primary coverage area includes Northbridge, Upton, Uxbridge, Mendon, Millville, Blackstone, and Bellingham. Territory is manageable with street parking and driveways available.
* EMR: Homecare Homebase.
* On call: None.
