Job description

Minimum Requirements
* Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
* Active Connecticut Physical Therapist license required to start the assignment.
* BLS certification from AHA or ARC may be required.
* Experience in home health settings preferred.
* Patient evaluation and documentation experience required, including home safety assessments and discharge planning.
* Ability to manage caseload independently and coordinate care with interdisciplinary teams.
