What You'll Be Doing

As a Physical Therapist, you'll provide individualized therapy in patients' homes and help them work toward safer, stronger, and more independent movement.

You'll: Evaluate patients and identify their physical therapy needs

Develop treatment plans based on each patient's condition and goals

Work on strength, mobility, balance, coordination, and functional movement

Provide hands-on therapy and therapeutic exercises

Monitor patient progress and adjust treatment as needed

Teach patients and families exercises and techniques they can continue at home

Help patients improve their ability to safely move around their homes and communities

Recommend appropriate equipment and mobility aids when needed

Communicate with physicians, families, caregivers, PTAs, and other members of the care team

Complete accurate documentation of evaluations, treatments, progress, and follow-up care

Participate in discharge planning and quality assurance activities

Supervise and provide direction to Physical Therapy Assistants (PTAs) when required

Provide education to patients, families, caregivers, staff, and the community

What We're Looking For

We're looking for a Physical Therapist who is: Licensed as a Physical Therapist in Ohio

A graduate of an approved Physical Therapy program

MS/DPT in Physical Therapy or BS with equivalent qualifying experience

Successfully completed the required state certification examination

Current with CPR certification

Committed to continuing education and professional development

A confident communicator with good interpersonal skills

Comfortable working independently with minimal supervision

Dependable, organized, adaptable, and patient-focused

Comfortable working with patients with physical, emotional, or cognitive challenges

Comfortable using computers for documentation and communication

Why Home Health?

No two patients-or two homes-are exactly alike.

Instead of seeing patients in the same clinic every day, you'll travel to their homes and work with them in the environment where they need to function.

You might help a patient improve their walking, practice getting up and down stairs, regain strength after an illness or injury, or learn how to move safely around their home.

It's therapy with a purpose: helping patients make progress they can actually use in their daily lives.

A Role With Independence and Support

You'll have the freedom to manage your patients and treatment plans while still having support from the therapy team and leadership.

We believe great therapy comes from combining clinical knowledge with patience, communication, and a genuine commitment to helping people improve.

The Details

This position involves frequent travel for home visits. Candidates must have: A valid Ohio Physical Therapist license

Current CPR certification

A valid driver's license

A safe and reliable vehicle

Current automobile insurance meeting Ohio requirements

A working phone and valid email address

Ability to safely perform patient transfers and other physical aspects of the position, with reasonable accommodation when applicable

Ready to Get Moving?

If you're a Physical Therapist who enjoys hands-on care, meaningful patient relationships, and seeing real progress from your work, we'd love to hear from you.

Apply today and help patients take their next step toward better movement and greater independence.
