nSchedule: Full Time- days, evenings
nPay Range: $36.00 - $46.00, based on experience.

n nJob Objective: Responsible for evaluation and treatment of individuals with sports and orthopedic conditions
nKey Responsibilities:·
nnnEvaluate and treat in outpatient setting.
n
nnDocumentation in accordance with state, federal, JCAHO and insurance guidelines as a per diem
n
nnProvide skilled outpatient Physical Therapy for Sports and Orthopedic conditions including evaluation, developing and implementing treatment programs.
n
n nRequirements:
nnnValid NYS licensure to practice Physical Therapy
n
nnDegree in Physical Therapy from CAPTE approved program, DPT preferred
n
nnPrior experience in outpatient care setting preferred
n
nnOrthopedic experience preferred
n
nnOCS, SCS, CSCS certifications a plus
n
nnExperience with EMR
n
nnTeam player and self-motivated
n
nnExperience or interest in sports/orthopedic patients desired.
n
n nPay Range: $36.00 - $46.00
nStarting Pay: Based on experience
