Assist patients in improving strength, mobility, balance, endurance, and functional independence. Implement individualized treatment plans in home health and skilled nursing environments. Monitor patient progress and communicate changes to the supervising Physical Therapist. Complete timely and accurate clinical documentation. Educate patients and caregivers on therapeutic exercises, mobility techniques, and safety precautions. Collaborate with physicians, nurses, therapists, and interdisciplinary care teams to support optimal patient outcomes.Qualifications Active, current, unrestricted Physical Therapist Assistant (PTA) license in the state of Georgia. Associate degree from an accredited Physical Therapist Assistant program. New graduates are encouraged to apply. Experience in home health, skilled nursing, rehabilitation, long-term care, or post-acute care is a plus but not required. Strong communication, documentation, and teamwork skills. Ability to provide compassionate, patient-centered care across a variety of care settings.
