Physical Therapy AssistantFairview Range is looking for a Physical Therapy Assistant to join our Hibbing Home Care and Hospice team. We're offering an up to $15,000 sign-on bonus! Minimum amount of $5,000 for a one-year commitment.We consider it an honor to provide care for our patients and their families and serve communities across the Iron Range. The RN Case Manager coordinates the delivery of all the care/services appropriate to the needs of the patients of all ages and their families using an interdisciplinary team approach. An individualized plan of care with specific goals is developed for each patient in collaboration with the physician.The physical therapy assistant is responsible for providing direct patient care in the home setting based on a treatment plan established by the Physical Therapist.Schedule: This position is 72-80 hours per two weeks, 8-hour Day shifts and no weekends.Fairview Range is an affiliate of M Health Fairview, a partnership of Fairview Health Services, the University of Minnesota, and M Physicians.
