nBrookhaven Health Care Facility is looking to add a Physical Therapist Assistant (PTA) to its rehabilitation team. In this role, you'll work under the supervision of a Physical Therapist to provide therapy services that help residents improve mobility, strength, balance, and overall function. nRESPONSIBILITIES:
nnnProvide skilled physical therapy treatments under the direction of a Physical Therapist
n
nnAssist with implementing individualized treatment plans designed to improve mobility, strength, balance, and functional independence
n
nnMonitor resident progress and communicate treatment updates to the supervising Physical Therapist
n
nnComplete required documentation, treatment records, and care plan updates accurately and timely
n
nnEducate residents, families, and staff on therapy techniques, exercises, mobility, and adaptive equipment
n
nnAssist with discharge planning, home exercise programs, and equipment recommendations
n
nnCollaborate with physicians, nursing staff, and interdisciplinary team members to support resident outcomes
n
nnParticipate in staff training, in-services, and quality improvement initiatives
n
nnMaintain therapy equipment, supplies, and treatment areas
n
nnPerform additional duties as assigned
n
n nREQUIREMENTS:
nnnAssociate Degree from an accredited Physical Therapist Assistant program approved by the American Physical Therapy Association
n
nnValid New York State Physical Therapist Assistant License required
n
nnNewly licensed Physical Therapist Assistants encouraged to apply
n
nnSkilled nursing facility experience preferred
n
nnStrong communication, documentation, and organizational skills
n
nnAbility to work collaboratively within an interdisciplinary healthcare team
n
nnCommitment to providing compassionate, resident-centered care
