Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our independent living community (Fountaingate Gardens) and our Certified Home Health Agency and Licensed Home Care Agency.

nWe are currently seeking a per diem Physical Therapy Assistant who will embody the passion and commitment required to deliver the ultimate care for our residents.
nEDUCATION AND EXPERIENCE REQUIREMENTS:
nnnAssociate degree from an approved PTA program or licensure.
n
nnTwo years of experience as a Physical Therapy Assistant preferred and 1 year geriatric experience preferred.
n
n nREQUIREMENTS INCLUDE: Under the supervision of the Physical Therapist, the Physical Therapy Assistant provides treatments to the resident to assist them in restoring their highest level of functioning.
nnnProvides treatments to the resident in accordance with the goals and plan set by the Physical Therapist.
n
nnModifies or adjusts treatment within the limits of the plan of care based upon resident’s reactions. Progresses treatment under the supervision of the Physical Therapist.
n
nnIndependently organizes a caseload that reflects the department’s productivity standard.
n
nnMaintains appropriate record keeping in accordance with facility and regulatory guidelines.
n
nnParticipates in the In-service and Continuing Education Programs of the Rehabilitation Department.
n
nnActs as Clinical Instructor, providing education and supervision to PTA students.
n
nnMaintains a working knowledge of job duties and departmental policies and procedures. Makes recommendations for revisions.
n
nnActively participates in the departmental Performance Improvement Program.
n
nnAssists in the planning, development or revision of programs within the Rehabilitation Department.
n
