Job description
VitalCaring is seeking a Physical Therapy Assistant to provide home-based rehabilitation services under the direction of a Physical Therapist. You will implement treatment plans, guide patients through therapeutic exercises, monitor progress, and report clinical changes to the care team. This role includes educating patients and families on safe mobility and use of assistive devices, documenting visits accurately in the EMR, and collaborating closely with nurses, therapists, and caregivers.
