n nPOSITION SUMMARY: Our School Based Physical Therapists serve students with various physical and developmental challenges, ranging in age from 3-21 years old.
nResponsibilities
nnnImplements treatment plans that focus on functional and measurable outcomes to be shared with the school and/or supervisors, as mandated in the student's Individualized Education Plans (IEPs).
n
nnConsults with other professionals (Teachers, SLP, OT, PCP, etc.) on the student's treatment team to ensure a cohesive and comprehensive therapy team.
n
nnAssesses and documents each student's progress through formal and informal tests and measurements.
n
nnWrites and prepares annual review reports and goals for each student in accordance with their IEP dates and requirements, in order to remain in compliance with state guidelines.
n
nnAttends all school related meetings as scheduled, such as, but not limited to, Triennials, Annual Reviews, Domains, Referrals, IEP, and pre-IEP meetings for each individual student.
n
nnProvides screenings and evaluations to determine requirements for skilled physical therapy intervention relative to the student/client's level of function within the school environment. This includes, but is not limited to the following areas: school function, range of motion, strength, positioning, contracture management, and conducting home assessments.

n nQualifications
nnnMust have a valid and unencumbered Physical Therapy license in the state(s) of practice, or proof of license and eligibility.
n
nnMust have a qualifying degree in Physical Therapy.
n
n nPosted Salary Range
nUSD $44.00 - USD $47.00 /Hr.
