Title: Physician – Post-Acute / Primary Care (Phoenix, AZ)

Summary: A leading healthcare organization is seeking a self-driven, compassionate Physician to join an expanding clinical team in the Phoenix market. In this role, you will provide high-quality care across assigned post-acute facilities, balancing direct patient management with Advanced Practice Provider (APP) mentorship. This position offers exceptional autonomy, daily schedule flexibility, and a highly rewarding productivity incentive structure for efficient providers.
Day to Day:
Perform initial history and physical examinations for new patients.
Conduct routine patient rounding (minimum once weekly) across designated post-acute facilities.
Partner with APPs to manage regulatory requirements and complex clinical visits.
Serve as a clinical mentor and resource to guide and elevate your APP team.
Proactively address quality gaps to drive measurable improvements in patient outcomes.
Act as a trusted advocate for patients and represent the organization’s mission within the local healthcare community.
Shift:
Monday – Friday, 8:00 AM – 5:00 PM (Standard hours with built-in flexibility and schedule autonomy across rotating facility locations in the Phoenix market)
Qualifications:
Education: MD or DO degree from an accredited medical school.
Board Certification: Board Certified in Internal Medicine, Family Medicine, Geriatrics, or Hospital Medicine.
Licensure: Active, unrestricted license to practice medicine in Arizona and an active DEA license.
Technical Skills: Proficiency with EMR systems.
Mobility: Comfort with multi-site travel across designated facility rotations within the Phoenix area.
Preferred: Prior experience or strong interest in Post-Acute, Long-Term Care, or Primary Care rounding.

Uncapped Productivity Incentives: Additional earnings of $30–$35 per RVU billed (ideal for providers comfortable seeing 15–20+ patients per day).
