Job description
Metropolitan Jewish Health System is seeking a Nurse Practitioner or Physician Assistant to provide and coordinate primary care for Advantage members in participating long-term care facilities and home settings in the Greater New York area.
You will develop care plans, coordinate preventive services, and serve as a clinical resource to providers and facilities, ensuring high-quality, patient-centered care for elderly populations.
