Job description
Hamilton Health Box is seeking an Advanced Practice Provider (APP) to work with supervising physicians across 3-5 primary care micro clinics. This role combines virtual and in-person care to deliver health coaching, disease management, preventive screening, and education to surrounding communities.
You will perform medical examinations, order tests, document care, provide injections and wound care, and develop patient-management plans while adhering to infection control and regulatory
