We are seeking a compassionate and experienced Primary Care Physician Assistant or Nurse Practitioner to join our dynamic team at Foundation Medical Partners / Southern NH Health. In this role, you will play a vital role in providing comprehensive care to a diverse patient population in Nashua, NH .

Collegial practice with solid APP support

W2, employed position with Foundation Medical Partners

40 Hours – 50% direct patient care work and 50% coordinating and communicating patient care

Clinical Affiliate of Massachusetts General Hospital

Hybrid Work Schedule – Potential remote options once fully trained

Qualifications:

Certified Physician Assistant or FNP

2+ years of Primary Care experience

Strong clinical skills,including assessment,diagnosis,and treatment planning.

Excellent communication,collaboration,and interpersonal skills.

Proficiency with EPIC EMR preferred

Ability to work independently and as part of a team.
